CH conducts physicals, clinic available August 7th

Blanchfield Army Hospital - Fort Campbell KYBlanchfield Army Community Hospital will conduct school and sports physicals for BACH prime enrolled beneficiaries during clinic hours until August 31st.

Network enrolled beneficiaries may schedule an appointment with their network provider for school physicals. All beneficiaries regardless of enrollment may schedule an appointment at BACH during the sports physical clinic which will be available August 7th in the Blue clinic from 8:00am until 4:00pm.

Beginning July 1st, the Tennessee Department of Health has new immunization requirements for children enrolling in child care and school. Children who start pre-school, pre-Kindergarten, Kindergarten, seventh grade, or are a new student starting school in Tennessee for the first time, will need a new official Tennessee Certificate of Immunization to provide to the school or child care provider.

Students will need to show proof of immunizations during school and sports physical exams.

Routine physical exams are valid for one year and are required for the following students:

  • any child entering Christian County or Clarksville-Montgomery County schools for the first time,
  • any student participating in school organized sports,
  • any youth participating in Fort Campbell’s organized sport programs,
  • any adolescent who enrolls in the sixth grade in Christian County or Fort Campbell Schools, and
  • any adolescent who is entering the seventh grade in Tennessee.

For children who attend schools other than the districts listed, parents are encouraged to call their local school districts to determine physical requirements and obtain necessary forms.

If children are involved in youth or school sports, physicals are required annually. Questions regarding school or sports physicals can be addressed with your clinic nurse. If your child had a school physical within the last 12 months, it will not be necessary to receive another physical.

Patients enrolled in a primary care clinic at BACH may contact their assigned clinic to have required school forms completed. Please allow three business days for the process to be completed.

To schedule a physical exam appointment at BACH, call the central appointments line at 931-431-4677 or 270-798-4677.

If for some reason you cannot make your scheduled school or sports physical appointment during the weekday, please call the BACH appointment line to cancel. If you need to cancel the Saturday appointment, call the Blue clinic front desk at 270-798-8764.

Hospital staff members request you bring your child’s school or sports physical examination forms provided by your child’s school and immunization records with you to the appointment. Children are asked to wear shorts and a short-sleeved shirt to facilitate the examination.
